You should be able to have good healing and exceptional shred with this build, of course you can't just run in... Move a little, poke with E and RMB and when the right time for you comes go in and burst their Mages and ADCs into hell.
Don't show yourself infront, your role is to flank! Never go in first!
There are some alternatives if you strife for anti heal or more anti tank. Just switch out the last 2 Items for the ones you like or need.
Don't forget, Anti shield might be important!!!
